Job summary

Chapters Health System, Inc. is seeking a Resource Nights & Weekend (ReNW) Nurse Practitioner to provide fully remote virtual serious illness care management to patients and families nationwide.

The role requires 3+ years of clinical experience in hospice, palliative, or acute inpatient settings, and a Masters/Doctoral degree in Nursing from an accredited NP program. Licensure, national certification, BLS, and compact RN status are required by hire.

Qualifications

• Must have 3+ years of recent clinical experience in hospice, palliative care, or acute inpatient care.
• Must hold a Masters or Doctoral degree in Nursing from an accredited Family or Adult NP program.
• Must hold national certification, licensure, and prescriptive authority for advanced nursing practice in home state and other care jurisdictions.
• Basic Life Support (BLS) certification and compact RN license in home state by date of hire.

Responsibilities

• Provides fully remote virtual serious illness care management to patients and families across our continuum of care.
• Consults with medical staff and IDT to ensure optimal patient care.
• Documents all interactions in the EMR and maintains electronic prescribing systems.
