Qualifications

Must have a Masters or Doctoral degree in Nursing from an accredited Family or Adult Nurse Practitioner Program.
Must hold national certification, licensure, and prescriptive authority for advanced nursing practice in home state and other required jurisdictions.
Basic Life Support (BLS) certification and compact RN license in home state by date of hire.

Responsibilities

Provides fully remote, virtual serious illness care management to patients and families across the continuum of care.
Consults with medical staff and IDT to ensure optimal patient care.
Performs virtual assessment of patient/family/caregiver needs.
Identifies, manages, orders, and prescribes medications and equipment as appropriate.
Supports discharge planning including prescribing medications and entering discharge orders.
