Chapters Health System, Inc. seeks a Resource Nights & Weekend (ReNW) Nurse Practitioner to deliver remote, virtual serious illness care management across our continuum of care.

The role supports home-based, facility-based, and inpatient hospice, palliative care, and related programs nationwide. Minimum 3 years of relevant clinical experience, MSN/DNP in Family or Adult NP, and active national certification with licensure and prescriptive authority.

Qualificações

• Minimum 3 years of clinical experience with hospice/palliative or acute inpatient care.
• Masters or Doctoral degree in Nursing from an accredited Family or Adult NP program.
• National certification, state licensure, and prescriptive authority for advanced nursing practice in home state and other states as applicable.

Responsabilidades

• Provide fully remote virtual serious illness care management to patients and families.
• Collaborate with medical staff to ensure optimal patient care.
• Perform virtual assessments and manage care plans.
• Prescribe medications and equipment as appropriate and within policy.
