Chapters Health System, Inc. is seeking a Resource Nights & Weekend (ReNW) Nurse Practitioner to deliver fully remote serious illness care management across home, facility, and inpatient settings.

The role requires 3+ years of clinical hospice/palliative experience, an NP Masters/Doctorate, and current national certification, licensure with prescriptive authority, BLS, and compact RN license by hire date.

Qualifications

• Minimum 3 years of recent clinical experience in hospice, palliative care, or acute inpatient care.
• Must hold a Masters or Doctorate in Nursing from an accredited NP program.
• National certification, licensure, and prescriptive authority in home state and others as required.
• Basic Life Support (BLS) certification and compact RN license by date of hire.

Responsibilities

• Provide fully remote virtual serious illness care management to patients and families across the continuum.
• Consult with medical staff to ensure optimal patient care and efficient team collaboration.
• Perform virtual assessments and manage medications, orders, and care plans.
• Document interactions in the EMR and support discharge planning with appropriate orders.
• Maintain HIPAA-compliant communication and participate in QAPI activities as needed.
