Chapters Health System is seeking a Resource Nights & Weekend Nurse Practitioner (ReNW NP) to provide fully remote serious illness care management to patients and families across our continuum of care, including hospice and palliative services.

The role requires a Masters or Doctoral degree in Nursing (Family or Adult NP), national certification, valid licensure, and prescriptive authority in home state and other states where care is provided.

Qualificações

• Masters or Doctoral degree in Nursing from an accredited Family or Adult NP program.
• National certification and prescriptive authority for advanced nursing practice.
• Compact RN license in home state by date of hire; Basic Life Support certification required.

Responsabilidades

• Provides fully remote virtual serious illness care management to patients and families across our continuum of care.
• Consults with medical staff to ensure optimal patient care and coordinate care.
• Performs virtual assessment of patient/family/caregiver needs.
• Identifies, manages, orders, and prescribes medications and equipment as appropriate.
• Adjusts and supports the plan of care according to policy and nurse practice act; documents in EMR.
