Chapters Health System, Inc. is seeking a Resource Nights & Weekend Nurse Practitioner (ReNW NP) to deliver remote serious illness care management.

You will support home-based, facility-based, and inpatient hospice and palliative care services across the continuum. The role requires 3+ years of clinical experience in hospice/palliative or acute inpatient settings, a Master’s/Doctorate in Nursing, and active licensure and prescriptive authority in relevant states.

Qualificações

• Requires a Masters or Doctorate in Nursing in Family or Adult NP program.
• National certification, licensure, and prescriptive authority in home state and others as needed.
• BLS certification and compact RN license in home state by hire date.

Responsabilidades

• Provides fully remote virtual serious illness care management to patients and families across Chapters Health System.
• Consults with medical staff to ensure optimal patient care.
• Performs virtual assessment of patient/family needs.
• Identifies, manages, and prescribes medications and equipment as appropriate.
• Adjusts the plan of care per Chapters policies and nurse practice act.
• Documents interactions in the EMR in a timely manner.
• Communicates with the primary team to ensure continuity of care.
• Supports discharge planning including prescribing meds and discharge instructions.
