Job summary

The Resource Nights & Weekend (ReNW) Nurse Practitioner (NP) position is a fully remote role within Chapters Health System. It focuses on virtual serious illness care management across home, facility, and inpatient settings, including hospice and palliative care services and related programs.

Applicants should have 3+ years of relevant clinical experience and hold a Master’s or Doctorate in Nursing with national certification and state licensure.

Qualifications

• Must have 3+ years of clinical experience in hospice, palliative care, or acute inpatient care.
• Must hold a Masters or Doctoral degree in Nursing from an accredited Family or Adult NP program.
• Must hold national certification, licensure, and prescriptive authority for practice in home state and applicable states.

Responsibilities

• Provide fully remote virtual serious illness care management to patients and families across care continuum.
• Consult with medical staff to ensure optimal patient care.
• Perform virtual assessments of patient/family needs and adjust care plans accordingly.
• Prescribe and manage medications/equipment as appropriate.
• Document interactions in the EMR promptly and accurately.
• Communicate with the primary team to ensure continuity of care.
