Job summary

The Resource Nights & Weekend (ReNW) Nurse Practitioner (NP) at Chapters Health System performs fully remote virtual serious illness care management across our continuum, including hospice and palliative care services. Requires a Master’s or Doctorate in Nursing, national certification, and current licensure with prescriptive authority; BLS and compact RN

Qualifications

• Minimum 3 years of clinical experience, including hospice/palliative/acute inpatient care.
• Master's or Doctorate in Nursing from an accredited Family or Adult NP program.

Responsibilities

• Provide fully remote virtual serious illness care management.
• Consult with medical staff to ensure optimal patient care.
• Perform virtual assessments of patient/family needs and adjust plans of care.
• Prescribe medications and equipment as appropriate within scope.
• Maintain EMR documentation and support discharge planning.
