Job summary

The Resource Nights & Weekend (ReNW) Nurse Practitioner (NP) at Chapters Health System is a fully remote position delivering serious illness care management to patients and families across our continuum, including hospice and palliative care services.

As part of our Medical Staff, you will provide virtual assessments, collaborate with the care team, and support discharge planning while maintaining HIPAA compliance and relying on EMR and e-prescribing systems.

Qualifications

• Must have a minimum of 3 years of recent clinical experience in hospice, palliative care and/or acute inpatient care.
• Must hold a Masters or Doctoral degree in Nursing from an accredited Family or Adult Nurse Practitioner program.
• Must hold national certification, licensure, and prescriptive authority in the home state and additional states where care is provided.
• Must hold and maintain Basic Life Support (BLS) certification and compact RN license in home state by date of hire.

Responsibilities

• Provides fully remote virtual serious illness care management to patients and families across the care continuum.
• Consults with medical staff and IDT members to ensure optimal patient care.
• Performs virtual assessment of patient/family/caregiver needs.
• Identifies, manages, orders, and prescribes medications and equipment as appropriate.
• Supports discharge planning by prescribing medications and entering discharge orders.
• Maintains access to EMR, e-prescribing systems, and related applications; documents interactions in EMR.
