The Resource Nights & Weekend (ReNW) Nurse Practitioner (NP) at Chapters Health System provides fully remote serious illness care management across home, facility, and inpatient settings. You’ll work with adult and pediatric patients in hospice, palliative care, and related service lines.

Expect shifts that may include nights, evenings, weekends, and holidays. Requires NP certification, MSN/DNP, and licensure in relevant states, with ongoing education and credentialing for care delivery.

Formación

• Must hold national certification as a Nurse Practitioner and be licensed with prescriptive authority in home state and other states served.
• Masters or Doctors degree in Nursing from an accredited Family or Adult NP program.
• Active RN license in home state and Basic Life Support certification required by date of hire.

Responsabilidades

• Provide fully remote virtual serious illness care management to patients and families.
• Consult with medical staff to ensure optimal patient care.
• Perform virtual assessments and manage medications/equipment.
• Enter orders and prescriptions; support discharge planning and care coordination.
• Maintain EMR access and timely documentation; support QA/QI activities.
