Job summary

Guidehealth is seeking a Healthguide to remotely support patients, bridging gaps between patients, physicians, and providers. You will coordinate care, educate patients, and help navigate medical and social services to improve outcomes.

Ideal candidates have 2–3 years in related roles, a Medical Assistant Certification, and strong EHR experience. This role offers remote work with occasional in-person collaboration and a language differential.

Qualifications

Fully bilingual in Arabic and English with medical terminology.
2–3 years in patient navigation, medical assistance, health promotion, or related roles.
Valid Medical Assistant Certification is required.
Experience with EHR documentation and remote work is preferred.
Willingness to perform other duties as assigned and adhere to policies.

Responsibilities

Build trusting, ongoing relationships with patients and care teams.
Engage high-risk patients using bidirectional communication to address needs.
Identify and assess medical, behavioral, social, and financial needs for whole-person care.
Educate patients on care plans and preventive needs; use motivational interviewing.
Document interactions and care activities in the EHR and Guidehealth systems.
Assist with referrals, transportation, scheduling, and linkage to specialists.
Escalate clinical concerns to RN Care Managers or leadership as needed.
Support population health initiatives and quality measure performance.
