CVS Health is seeking a Case Management Coordinator for a full-time telework role with travel in select Illinois counties. The position supports members through scheduling, benefits education, and care management tools to achieve optimal health outcomes.

Hours are 8:00 am–5:00 pm CT, Monday–Friday, with up to 75% travel. The role emphasizes evaluation, care planning, collaboration with clinicians, and member engagement to improve care quality and access.

Formación

• 2 years+ experience in behavioral health or social services.
• 1+ year(s) of experience with Microsoft Office Suite applications (MS Word, Excel, Outlook and PowerPoint).
• Willing and able to travel 50-75% in the specified counties; reliable transportation required.

Responsabilidades

• Evaluate members' needs/eligibility and recommend an approach to care.
• Coordinate and monitor assigned care plan activities and progress.
• Identify high risk factors and refer to clinical case management as appropriate.
• Engage with team members and contribute to care conferences to optimize outcomes.
• Provide coaching and support to empower members in medical and lifestyle choices.
