Responsabilités

• Evaluate member needs and eligibility to inform care plan.
• Identify high-risk factors and coordinate referrals to clinical teams.
• Coordinate and monitor care plan activities.
• Collaborate with Medical Directors and other health programs to optimize outcomes.
• Document care activities in compliance with regulatory guidelines.
