HCSC Group is seeking a Government Programs Care Manager III to coordinate care for members remotely from home across the United States. The role includes telephonic and field-based activities, health screenings, and comprehensive assessments to address health needs and goals.

Qualified candidates hold an RN or other eligible licenses with clinical experience, and may travel within the assigned territory. Bilingual English/Spanish is a plus.

Qualifications

• RN/LCPC/LCSW/LMFT with 2 years clinical practice required.
• Current valid license in the state of operations.
• 3 years wellness or managed care experience.
• Excellent verbal and written communication.
• Ability to travel within assigned territory.
• Bilingual in English and Spanish preferred.

Responsibilities

• Provide telephonic and field-based care coordination.
• Conduct health screenings and comprehensive health assessments.
• Develop and implement care plans for complex populations.
• Engage members to discuss adherence to health goals and education.
• Support clinical operations with provider and member activities.
