HCSC is hiring a Government Programs Care Manager III to coordinate care for members remotely across IL. The role combines telephonic and field-based activities, including health screenings and comprehensive assessments, with travel to member homes or facilities when needed.

Ideal candidates hold an unrestricted RN, LCPC, LCSW, or LMFT license and have 3+ years in wellness or managed care, strong communication, and proficiency with Word, Excel, and PowerPoint.

Qualifications

• RN or LCPC/LCSW/LMFT with 2 years of clinical practice in a professional setting.
• Active, valid and unrestricted license in the state of operations or reciprocity; for compact licensees, obtain active RN licensure within 90 days.
• 3 years wellness or managed care experience with clinical issues involving members/physicians.
• Strong communication, analytical, and computer skills (Word, Excel, PowerPoint).
• Willingness to travel within the assigned territory.

Responsabilités

• Provide telephonic and field-based care coordination to inform and educate members about health care programs.
• Conduct health screenings, assessments, and care planning; may visit homes or facilities.
• Engage members in adherence to personal health goals and coordinate with providers and internal teams.
• Interact with external vendors and community resources to support member care.
• Support clinical operations with documentation and data analysis.
