Job summary

Health Care Service Corporation seeks a dedicated care coordination professional to support members in New Mexico via telephonic and field-based care management. You will educate members on health programs and help them adhere to personal health goals, with potential home visits for complex cases.

The role requires 2+ years of clinical or wellness care experience, NM licensure, and occasional travel within the territory; standard hours are 8 AM–5 PM MST, Monday–Friday.

Qualifications

• Current, valid, unrestricted license in the state of New Mexico.
• 2 years direct clinical care experience.
• 3 years wellness or managed care experience.
• Strong verbal and written communication skills.
• Willingness to travel within assigned territory.

Responsibilities

• Provide care coordination to members via telephonic and/or field-based care management.
• Educate members on health care programs to address personal health needs.
• Complete health screenings, comprehensive health assessments and care planning.
• May require visits to members' homes and/or facilities to support complex populations.
