Job summary

HCSC in Santa Fe, NM is seeking a Care Coordination Nurse/Clinician to deliver telephonic and field-based care management. You will educate members on health programs, discuss personal health goals, respond to inquiries, and support clinical operations with provider and member activities.

This field-work role requires 2–3 days travel to Dona Ana and nearby areas; remote work from NM is available. You may complete health screenings, assessments, and care plans, with 8 AM–5 PM MST hours;

Qualifications

• RN with 2 years direct clinical care in a clinical setting, or unrestricted LPC/LMSW/LCPC/LCSW/LMFT with 2 years clinical practice.
• Current, valid, unrestricted NM license (or reciprocity); NM RN licensure within 90 days for compact licensees.
• Plus 3 years’ wellness or managed care experience presenting clinical issues with members/physicians.
• Knowledge of the health and wellness marketplace and employer trends.
• Verbal and written communication with members; interfacing with internal staff/external vendors.
• Analytical experience including medical data analysis.
• PC proficiency to include Word, Excel, and PowerPoint; database experience and Web-based applications.
• Current unrestricted driver’s license, transportation and applicable insurance.
• Ability and willingness to travel within assigned territory.

Responsibilities

• Provide care coordination to members via telephonic and field-based care management.
• Educate members on health care programs to address personal health needs.
• Engage members in discussion of adherence to personal health goals.
• Respond to member inquiries and support clinical operations with provider and member activities.
• Complete health screenings, health assessments and care planning; may involve home or facility visits.
• Travel within assigned territory as required.
