Job summary

HCSC is seeking a registered nurse or licensed clinician to provide care coordination to members via telephonic and field-based care management. You will educate members on health care programs, discuss goals, and support clinical operations with provider activities.

This role may include home or facility visits to support complex populations; travel within the assigned territory is required. Base pay range is $34.90–$57.89 per hour, and remote work options are available across the continental

Qualifications

• RN with 2 years direct clinical care in a clinical setting.
• Licensed professionals: LCPC, LCSW, or LMFT with 2 years clinical practice.
• Current, valid, unrestricted license in the state of operations.
• For compact licensees, obtain active RN licensure in state of operations within 90 days of hire.
• Plus 3 years wellness or managed care experience.
• Knowledge of the health and wellness marketplace and employer trends.
• Verbal and written communication skills.
• Analytical experience with medical data.
• PC proficiency to include Word, Excel, and PowerPoint; database and web applications.
• Current unrestricted driver’s license; ability to travel.

Responsibilities

• Provide care coordination to members via telephonic and/or field-based care management.
• Inform and educate members on health care programs.
• Engage the member in discussion of adherence to personal health goals.
• Respond to inquiries from members.
• Support clinical operations with provider and member activities.
• Complete member telephonic and/or field-based health screenings, comprehensive health assessments and care planning duties.
• May require visits to members' homes and/or facilities to support complex populations.
