Job summary

Health Care Service Corporation is seeking a field- and telephonic care coordinator to support members across the continental United States, with potential home or facility visits for complex populations. The role requires a current RN, LPC, LMSW, LCPC, LCSW, LMFT, LPCC, LISW or related license and at least 2 years of clinical experience.

Responsibilities include health screenings, comprehensive assessments, and coordinated care planning, working with providers and internal teams to address

Qualifications

• Must hold an active, unrestricted RN license in the state of operations or reciprocity.
• Plus 3 years’ wellness or managed care experience with clinical issues
• Strong verbal and written communication with members and internal/external partners
• Analytical data analysis experience and proficiency with Word, Excel, and PowerPoint

Responsibilities

• Provide care coordination to members via telephonic and/or field-based care management.
• Educate members on health care programs to address personal health needs.
• Complete telephonic and/or field-based health screenings and comprehensive health assessments.
• Perform care planning duties which may require home or facility visits to support complex populations.
