Job summary

UnitedHealth Group is seeking an ECF CHOICES Support Coordinator to manage a panel of members with intellectual or developmental disabilities. This home-based role requires travel up to 75% to meet with members and providers, focusing on comprehensive assessments and person-centered care plans.

You will collaborate with internal teams and community resources to address medical, behavioral, and social needs, promoting quality care and optimal service utilization.

Qualifications

• Must have 4-year or higher degree in social work, nursing, education, rehabilitation counseling, or another human service field.
• RN or LPN license.
• QDDP or QIDP certification per federal requirements.

Responsibilities

• Conduct comprehensive needs assessments with medical, behavioral, functional, cultural, and socio-economic factors.
• Develop and implement person-centered care plans addressing chronic conditions, health promotion, and social determinants of health.
• Coordinate care with internal teams and community partners to execute the care plan.
• Provide referrals and linkage to housing, pharmacy, peer support, and other resources as appropriate.
