Qualifications

• Current and unrestricted Idaho license in one of the listed clinical roles (RN, LCSW, LMSW, LSW, LCPC, LPC, LMFT, LAMFT)
• 1+ years of experience with Medicaid/Medicare or SDoH needs
• 1+ years of experience with MS Office (Word, Excel, Outlook)
• Driver's license and ability to travel up to 50% within assigned territory
• Remote-friendly/home workspace compliant with PHI security
• Residence in Idaho with access to high-speed internet

Responsibilities

• Serve as the primary care manager for a panel of members with chronic and complex health care needs
• Conduct comprehensive needs assessments and develop person-centered care plans
• Collaborate with internal care team and community resources to implement care plans
• Assist members with HCBS supports and services
• Provide referrals and linkage to appropriate internal and external resources
• Support discharge planning and care transitions after ER or facility stays
• Educate and coach members for self-management and lifestyle changes
• Advocate for member needs within the health care team
• Address nonclinical questions with providers and facility staff
