UnitedHealthcare is seeking a Field Care Coordinator to support members in Rutherford County, TN, with the ECF CHOICES program. This role is home-based with 75% field work and requires travel within advertised counties.

You will assess medical, behavioral, and social needs and develop individualized care plans aligned with evidence-based guidelines, coordinating with internal teams and community providers to improve health outcomes.

Qualifications

• RN or LPN license (or equivalent) required or 5+ years as a support coordinator/case manager.
• Four-year degree in human services or healthcare field or related credentials.
• Qualified Developmental Disabilities Professional (QDDP) or QIDP certification preferred.
• Reside within 50 miles driving distance to Williamson County, TN.

Responsibilities

• Conduct comprehensive needs assessments for members across medical, behavioral, and social needs.
• Develop and implement person-centered care plans with goals and interventions.
• Collaborate with care team, providers, and community resources to execute care plans.
• Provide referrals and linkage to internal and community-based services.
