Job summary

UnitedHealth Group is seeking a dedicated ECF CHOICES Support Coordinator to manage a panel of members with intellectual or developmental disabilities. The role blends in-person and remote work, requiring travel up to 75% within Tennessee counties to coordinate care and resources across medical, behavioral, and social needs.

You will engage members to assess comprehensive needs, develop person-centered plans, and connect with internal teams and community resources to improve quality of life.

Qualifications

• 4-year or higher degree in social work, nursing, education, rehabilitation counseling, or other human service or health care profession
• RN or LPN licensure
• QDDP or QIDP certification
• 5+ years as independent Support Coordinator or HCBS case manager with required trainings
• Valid driver's license and travel up to 75% within counties to meet members/providers
• Reside within 50 miles of Knox County, TN

Responsibilities

• Engage members face-to-face or by phone to complete needs assessments across medical, behavioral, functional, cultural and social determinants of health
• Develop and implement person-centered care plans addressing chronic conditions, wellness, SDoH, medications, safety
• Coordinate with internal care team, providers, and community resources to execute care plans
• Provide referrals/linkages to housing, pharmacy, peer support, PCPs or specialists as appropriate
• Adhere to telecommuter policy for remote work as applicable
