Job summary

UnitedHealthcare is seeking a Care Coordinator to manage a panel of members with behavioral health needs, completing Health Risk Assessments and Individualized Service Plans while linking members to needed services.

The role includes field visits 25-50% of the time, collaborating with physicians and community partners, and a field-based/home-office setup with remote work options for residents near Northern Virginia.

Qualifications

• Bachelor's degree in health or human services field OR LMHP OR RN/LPN OR QMHP OR LMSW OR LBSW OR MSW OR BSW.
• 1+ years of care coordination or behavioral health experience and/or work in a healthcare environment.
• 1+ years of experience with MS Office, including Word, Excel, and Outlook.
• Driver's license and reliable transportation and the ability to travel within assigned territory to meet with members and providers if required.

Responsibilities

• The Care Coordinator will be the primary care manager for a panel of serious mental illness/SMI members with varying risk and may be assigned other health plan populations as needed.
• This position is responsible for completing Health Risk Assessments, Individualized Service Plans and linking members to needed services to ensure that the members are receiving the healthcare they need.
• This position would involve working with physicians, behavioral health providers, and other community stakeholders as well. Care Managers working with this population will be required to work in the field to complete face-to-face visits for 25-50% of assigned members.
• Care coordination activities will focus on supporting members' medical, behavioral, and socioeconomic needs to promote appropriate utilization of services and improved quality of care.
