UnitedHealthcare, part of UnitedHealth Group, is seeking an ECF CHOICES Support Coordinator to help members with intellectual or developmental disabilities. You will assess medical, behavioral, and social needs, develop person-centered care plans, and collaborate with care teams to improve access to services and quality of care.

This is a home-based role with about 75% field work; candidates in the Dixon, Montgomery, and West Davidson Counties area may work remotely while traveling to meet

Qualifications

• 4-year or higher degree in social work, nursing, education, rehabilitation counseling, or other human service or health care profession.
• RN or LPN licensure per requirements.
• QDDP or QIDP certification per federal standards.
• 5+ years independent Support Coordinator experience for HCBS waivers.

Responsibilities

• Engage members face-to-face or by phone to assess comprehensive needs (medical, behavioral, functional, cultural, SDoH).
• Develop and implement person-centered care plans addressing chronic conditions, wellness, and safety.
• Collaborate with internal teams, providers, and community resources to implement care plans.
• Provide referrals and linkages to appropriate internal or community providers as needed.
• Support and document progress to promote quality of care and service utilization.
