Responsibilities

• Administer LTSS assessments through person-centered thinking.
• Contact members to establish goals and priorities and develop plan of care.
• Develop and modify Service Plan with care team involvement.
• Assist members in navigating LTSS and related needs.
• Support access to long-term services regardless of funding sources.
