Job summary

Humana Inc. is seeking a Care Coordinator to support Illinois Medicaid Members in the Humana Gold Plus Integrated program. You will assess needs, develop personalized care plans, and connect members with resources to improve health and well-being.

You will report to the Manager, Care Management and may support MLTSS or LTSS as needed. This field role requires 75-90% travel to visit members, work Mon-Fri 8am-5pm CST, and compliance with Humana safety programs.

Qualifications

• Must reside in Illinois.
• 1+ years of health care or case management experience.
• 1+ years of experience with Medicare & Medicaid recipients and/or long-term care or home and community-based services.

Responsibilities

• Visit Medicaid members in homes, facilities, and care settings with 75-90% local travel.
• Assess care needs to ensure services in least restrictive settings.
• Identify and resolve barriers hindering care.
• Plan and implement interventions to meet needs.
• Coordinate services and monitor the care plan against goals.
• Monitor patient progress toward outcomes and adjust care.
• Assist enrollees with grievances and appeals.
