Humana is seeking a Field Service Coordinator (Care Coach 1) to assess members’ needs and guide them toward appropriate resources to promote wellness. You will meet members in their location, spend time evaluating barriers, and connect them with services to improve health outcomes.

Responsibilities include LTSS assessments, care planning, and collaboration with the care team to ensure holistic service delivery.

Competenze

• Bachelor’s degree in health and human services field.
• 2+ years of related experience.
• Proficient in Microsoft Word and Excel.
• Excellent verbal and written communication skills.
• Able to use a variety of electronic information applications/software.
• TB screening required for this role.
• Must have a valid state driver’s license and maintain personal vehicle liability insurance.
• Must have a separate room with a locked door for home office privacy.

Mansioni

• Administer ongoing LTSS assessments using person-centered thinking.
• Contact members to establish goals and plan of care.
• Develop and modify Service Plans with care team.
• Assist members in navigating LTSS and related needs.
• Support access to housing, education, and other services.
• Assist with Medicaid eligibility requirements.
• Collaborate with Medical Director, Geriatrician, or Care Coordinator.
