Job summary

Humana Inc. is seeking a Field Service Coordinator (Care Coach 1) to assess member needs and guide them to LTSS resources, working remotely in Indiana. You will meet members, develop plans of care, and coordinate with care teams to promote wellbeing and outcomes.

The role emphasizes person-centered planning, Medicaid eligibility support, and collaboration with Medical Directors, geriatricians, and coordinators to deliver holistic services. Travel to Humana offices may be required for training.

Qualifications

• Bachelor's degree in health and human services field.
• 2+ years of related experience.
• Knowledge of Microsoft Office Word and Excel.
• Exceptional verbal/written communication and interpersonal skills.
• This position must reside in one of Indiana's Cass, Fulton, Howard, Miami, Tipton, or Wabash counties.

Responsibilities

• Administer LTSS assessments and develop plans of care using person-centered thinking.
• Contact members to establish goals and prioritize resources, including LTSS providers.
• Develop and modify service plans with care team involvement (Informal caregiver coach, PCP, etc.).
• Support members through navigation of LTSS and related needs across services.
• Assist members in maintaining Medicaid eligibility and coordinating with resources.
• Collaborate with Medical Director/Geriatrician/Care Coordinator to ensure cohesive care.
