Job summary

Humana is seeking an RN Field Care Manager 2 (Physical Health) to assess member needs and guide families toward resources for optimal wellness. You will perform telephonic and in-person assessments, develop personalized care plans, coordinate with providers, and monitor progress to achieve individual health goals.

Based in Louisiana, you will report to the Manager of Care Management within the Louisiana Medicaid Team and travel up to 50% within the region to support members and providers with

Qualifications

• Active, unrestricted RN license in Louisiana.
• 2+ years clinical experience with diverse populations.
• Knowledge of community health and social service agencies.
• Experience with electronic medical records and narrative documentation.
• Proficient in MS Word, Outlook, and Excel.
• Ability to work independently and adapt care plans.

Responsibilities

• Conduct telephonic and in-person assessments to evaluate member needs and guide families toward resources.
• Develop and implement personalized care plans based on clinical judgement and psychosocial factors.
• Collaborate with healthcare providers and community organizations to coordinate care.
• Monitor member progress and adjust care plans to meet goals.
• Document incidents and maintain compliance with policies and regulations.
