Job summary

Humana Healthy Horizons in Indiana seeks a Care Coordinator 2 (Field Care Manager 2) to assess members' needs and guide families toward resources to achieve optimal wellness. This role supports Indiana PathWays for Aging, helping seniors age at home with improved access to services.

You will be part of a caring Humana community, with hybrid work, field visits, and collaboration with the Interdisciplinary Care Team to coordinate medical and non-medical services for members.

Qualifications

• RN license in Indiana (unrestricted).
• 2+ years clinical experience in case management or similar health care services.
• Proficient with MS Word, Excel, Outlook; strong computer skills.
• Excellent communication and interpersonal skills; able to manage multiple priorities.

Responsibilities

• Coordinate member care across medical and non-medical services.
• Develop longitudinal, trusting relationships with members to improve outcomes.
• Collaborate with ICT and other care team staff to address identified needs.
• Coordinate with Medicare payers and providers for eligible members.
