Job summary

Humana is seeking a Senior Care Coordinator to support a team in delivering patient-centered care. The role involves field visits (75-90% travel) to Medicaid/Long-Term Care members, coaching, mentoring, and ensuring care plans meet member goals.

Requires a healthcare background with case management experience, plus licensure (LPN/LPC/LSW) and Illinois residency. Bonus eligible and field-friendly schedule.

Qualifications

• Bachelor's degree required in social sciences, social work, human services or related field.
• 2+ years of health care and/or case management experience.
• Experience with Medicare & Medicaid recipients and/or long-term care.
• Ability to use electronic information applications including electronic medical records.

Responsibilities

• Provide coverage for Care Coordinators and document in the clinical system.
• Mentor and train team members and support training initiatives.
• Submit daily reports to Care Coordinators on outstanding cases and review compliance dashboards.
• Support audit activities and internal/external audits for the team.
• Complete field work support and review reports.
• Assist with onboarding of new hires including precepting and case audits.
• Coordinate services and monitor the care management plan against member goals.
• Ensure patients progress toward outcomes through assessment, data, conversations, and care planning.
