Job summary

US Health Partners, LLC seeks a Nurse liaison to coordinate care and communicate medical information among the Injured Worker, the Insured, and the Carrier to facilitate treatment, rehabilitation, and return to work. This is a remote, field-based role with regular local travel up to 2 hours in your region.

Responsibilities include following the CM plan, scheduling assessments, documenting data in CaseAnyplace, and coordinating referrals to Vocational Counselor and IMEs.

Qualifications

• Current, unrestricted state license as a Registered Nurse.
• Eligibility to practice in states where field case management is provided.
• Completion of a nursing program with ongoing CE requirements.

Responsibilities

• Follow the Case Management plan and retain accountability for the CM process.
• Accept referrals and schedule initial assessments per CM plan.
• Maintain ongoing communication with Injured Worker, Insured, and Carrier; coordinate care and RTW planning.
