Job summary

US Health Partners, LLC seeks a Nurse liaison to coordinate care and communicate important medical information among the Injured Worker, the Insured, and the Carrier to facilitate treatment, rehabilitation, and return to work. This is a remote, field-based role with regular local travel up to 2 hours in your region.

You will follow the Case Management plan, accept referrals, schedule assessments, document data, and coordinate care with physicians, therapists, and vocational resources while

Qualifications

• Current, unrestricted RN license; licensed in each state where field case management is provided.
• CE credits ongoing as required by state practice.
• Certification such as CCM/CRC/CLNC/CRRN within three years of hire or state timeframe.

Responsibilities

• Coordinate care among Injured Worker, Insured, and Carrier to facilitate treatment and return to work.
• Follow CM plan and accept referrals from Director of Case Management.
• Record data and billing; submit monthly reports.
• Attend physician appointments and coordinate IMEs and vocational assessments as appropriate.
• Coordinate transportation and translation as needed, and promote teamwork.
