Job summary

Opus Medical in the United States seeks a nurse liaison to coordinate care and share essential medical information among the Injured Worker, the Insured, and the Carrier to enable appropriate treatment, rehab, and return to work. This is remote, field-based with local travel up to 2 hours in your region.

Duties include following the Case Management plan, taking referrals, scheduling assessments, documenting data, and supporting RTW planning.

Qualifications

• Completion of a nursing program and ongoing CE as required.
• Active nursing license in needed state(s) with eligibility for independent assessment.
• Two years of direct case management experience preferred.

Responsibilities

• Follow Case Management plan and maintain CM process accountability.
• Accept referrals as assigned by the Director of Case Management.
• Schedule initial assessments and obtain dates for physician appointments.
• Maintain ongoing communication with Injured Worker, Insured, and Carrier.
• Record data and billing; submit monthly reports.
• Attend physician appointments to obtain diagnosis, prognosis, and treatment plan.
• Coordinate RTW planning and liaise with Vocational Counselor when appropriate.
• Provide translation as needed and promote teamwork with staff.
