Job summary

US Health Partners, LLC seeks a Nurse Liaison to coordinate care and communicate medical information among the Injured Worker, the Insured, and the Carrier to facilitate appropriate treatment and return to work. This remote, field-based role involves regular local travel up to 2 hours within your region.

You will follow CM plans, accept referrals, schedule assessments, and maintain ongoing communication with all parties while ensuring timely data entry and reporting.

Qualifications

• Current, unrestricted registered nurse license; licensed in each state where field case management is provided.
• Ability to perform independent assessments within scope of practice.
• Completion of nursing program and ongoing continuing education as required.
• Must obtain and maintain at least one nationally recognized certification (e.g., CCM, CRC, CLNC, CRRN) within three years of hire.
• Two years FTE direct case management experience for injured workers preferred.

Responsibilities

• Coordinate care and communicate medical information among the Injured Worker, the Insured, and the Carrier.
• Attend physician appointments; document diagnosis, prognosis, treatment plan, rehab length, RTW status, and MMI/Full Recovery.
• Record data and billing in CaseAnyplace; submit timely monthly reports.
