Job summary

Reema Health is seeking a Community Health Navigator to engage with members remotely and in the field, guiding them toward better health outcomes. The role focuses on building genuine, supportive relationships, addressing social barriers, and coordinating care within Reema’s technology platform.

You will enroll members, manage a caseload, assess needs, and help them engage with community resources. This position requires adaptability in a fast-paced startup environment with occasional in-person

Qualifications

Responsibilities

• Conduct phone and in-person outreach and enroll members in Reema's care model.
• Build and manage a caseload of 80-120 members and cover for others as needed.
• Assess member needs to develop a member-centered care plan.
• Coordinate referrals and schedule appointments.
• Document work in the Reema App and maintain HIPAA compliance.
• Hit monthly goals and daily metrics.
