Job summary

Reema Health is seeking a Community Health Navigator to build genuine relationships with members and guide them toward greater engagement in their healthcare. You will work through Reema's technology daily to reach members, schedule visits, and document progress in a remote field-based role.

You will assess social determinants of health, coordinate referrals, and support monthly goals with a flexible startup mindset.

Qualifications

• 18 years of age or older.
• Available to work Monday–Friday during core business hours.
• Valid driver's license, insurance, and reliable transportation
• Offers contingent upon passing a pre-employment drug screening and background check
• Comply with federal, state, and local regulations regarding patient confidentiality and HIPAA

Responsibilities

• Conduct outreach and enroll members into Reema's care model.
• Build a caseload of 80-120 members and cover others during spikes.
• Assess member needs to build a member-centered care plan.
• Coordinate referrals and schedule appointments.
• Document all member work in the Reema App and maintain confidentiality.
