Job summary

Reema Health seeks a Community Health Navigator to build genuine relationships with members and guide them toward greater engagement in their health care. You will work through Reema's tech platform to reach members, schedule visits, and document progress in a growing startup environment.

You will enroll members in the care model, manage a caseload, assess needs, and coordinate referrals while maintaining HIPAA confidentiality and professional boundaries.

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, Health Sciences, or related field (or equivalent direct experience).
• Experience with crisis planning, case management, and professional bounds in community-based work.
• 18 years of age or older.
• Use technology-based tools and systems daily.

Responsibilities

• Conduct outreach and enroll members in Reema's care model.
• Build and manage a caseload (80-120 members) and cover for others during peaks.
• Assess member needs, including social determinants of health, to build care plans.
• Establish trusting relationships and follow through in a timely manner.
• Coordinate referrals and scheduling; hit monthly goals and daily metrics.
