Job summary

Reema Health seeks a Community Health Navigator to engage with members remotely and in the field, guiding them toward greater engagement in their healthcare. You will use Reema's technology platform to reach members, schedule visits, and document work, with growth opportunities as the startup evolves.

This role emphasizes relationship-building, addressing social barriers, and collaborative care within a multidisciplinary team to improve wellbeing and health outcomes.

Qualifications

• 18 years of age or older.
• Available to work Monday–Friday during core business hours.
• Valid driver’s license, insurance, and reliable transportation.
• Contingent upon passing a pre-employment drug screen and background check.
• Comply with HIPAA and patient confidentiality requirements.

Responsibilities

• Conduct phone and in-person outreach and enroll members in Reema's care model.
• Build and manage a caseload (80–120 members) and cover for others during spikes.
• Assess member needs and develop member-centered care plans.
• Coordinate referrals and schedule appointments.
