Reema Health is seeking a Community Health Navigator to build authentic member relationships and guide them toward greater engagement in their healthcare. You will use Reema's platform daily to reach members, schedule visits, and document work while traveling within a 25–50% range for in-community services.

Your role emphasizes meeting community needs, coordinating referrals, and supporting members in overcoming barriers with empathy. This is a growing startup where roles evolve rapidly.

Qualifications

• Experience with crisis planning, case management, and professional boundaries in community-based work.
• Strong communication and relationship-building skills.
• Comfort using technology-based tools daily.
• Ability to work remote and manage a caseload.
• Lift and carry up to 25 lbs occasionally.

Responsibilities

• Conduct outreach and enroll members in Reema's care model.
• Build and manage a caseload (80-120 members) and cover for others as needed.
• Assess member needs and develop member-centered care plans.
