Job summary

Reema Health is seeking a Community Health Navigator to build trusted relationships with members and guide them toward better engagement in their healthcare. This remote/field-based role requires coordinating visits, documenting progress, and leveraging Reema's platform to reach clients in the Georgetown, KY area.

You will enroll members, manage a caseload, assess social determinants of health, and help arrange referrals and appointments while upholding confidentiality and team collaboration.

Qualifications

• Collaborative mindset for a remote environment with occasional in-person meetings.

Responsibilities

• Outreach via phone and in-person to enroll members in Reema's care model.
• Build and manage a caseload of 80-120 members and cover for others when needed.
• Assess needs to create member-centered care plans and coordinate referrals.
• Schedule and track appointments; document work in the Reema App.
• Maintain HIPAA confidentiality and professional boundaries.
