Qualifications

• Strong verbal and written communication skills.
• Experience with remote case management tools.
• Ability to navigate social determinants of health.
• Comfort with HIPAA and confidentiality.

Responsibilities

• Conduct phone and in-person outreach and enroll members in Reema's care model.
• Build and manage a caseload (80-120 members), providing coverage for other Guides during absences or workload spikes.
• Assess member needs, including social determinants of health, to build a member-centered care plan.
• Establish trusting relationships and follow through in a timely, appropriate manner.
• Coordinate referrals and support appointment scheduling and attendance.
