Job summary

Reema Health is seeking a fully remote/field-based Community Health Navigator in the Cleveland area to build relationships with members and guide them toward greater engagement in their healthcare. You will operate through Reema's platform, schedule visits, and document progress as part of a growing startup.

Qualifications

• Solutions oriented and able to improvise a path forward when barriers arise.
• Strong verbal and written communication skills.
• Comfort with learning and using new tools on mobile and desktop.
• Self-motivated to stay productive on a remote caseload.
• Team collaboration in a remote environment and occasional in-person meetings.

Responsibilities

• Conduct outreach by phone and in-person to enroll members in Reema's care model.
• Build and manage a caseload of 80-120 members and cover for others as needed.
• Assess member needs and develop member-centered care plans.
• Establish trust and follow through in a timely, empathetic manner.
• Help members understand health activities and drive completion.
• Coordinate referrals and schedule appointments; track attendance.
