Job summary

Reema Health is seeking a Community Health Navigator to enroll members, build relationships, and guide them toward engagement in their care. You will work with a remote caseload, document activities in the Reema App, and coordinate referrals while maintaining HIPAA standards.

The role emphasizes compassionate outreach, teamwork with a multidisciplinary staff, and adaptability in a growing startup environment.

Qualifications

• 18+ years of age or older
• Available to work Monday–Friday during core business hours
• Driver's license, insurance, and reliable transportation
• Pass pre-employment drug screening and background check
• Comply with HIPAA and patient confidentiality rules
• Ability to use technology-based tools daily
• Lift and carry up to 25 lbs occasionally

Responsibilities

• Conduct phone and in-person outreach and enroll members in Reema's care model
• Build and manage a caseload (80-120 members)
• Assess member needs and build member-centered care plans
• Coordinate referrals and schedule appointments
• Document all member work in the Reema App and maintain care plans
