Reema Health, Inc. seeks a Community Health Navigator to build trusted relationships with members, guiding them toward greater engagement in their healthcare and addressing social barriers through Reema's technology platform.

The role is remote/field-based, with 25–50% travel in the local area. You will schedule visits, document work, and coordinate referrals while maintaining confidentiality and HIPAA standards.

Qualifications

• Comfort learning and troubleshooting new tools on mobile and desktop.
• Self-motivated to stay productive on a remote caseload.

Responsibilities

• Conduct outreach and enroll members in Reema's care model.
• Manage a caseload and provide coverage for other Guides as needed.
• Assess member needs to build a member-centered care plan.
• Coordinate referrals and schedule appointments.
• Document all member work in the Reema App and maintain HIPAA compliance.
