Job summary

Enlyte is seeking a Field Case Manager RN in the Vero Beach, FL area to coordinate care for injured workers. You will work with physicians, employers, and patients to develop and monitor treatment plans and ensure timely, cost-effective care.

This full-time role travels regularly to in-person appointments; a sign-on bonus is offered and comprehensive benefits. You will independently manage caseloads, document progress, and support return-to-work goals.

Qualifications

• Active RN license in good standing.
• URAC-recognized certification in case management (CCM, CDMS, CRC, CRRN or COHN, COHN-S).

Responsibilities

• Coordinate and implement treatment plans with physicians, employers, and patients.
• Collaborate with treaters to modify job duties for early return to work.
• Manage caseloads independently and provide status updates to stakeholders.
• Ensure access to cost-effective healthcare services and monitor patient progress.
• Attend in-person appointments as required and document outcomes.
