Job summary

Enlyte is seeking a Field Case Manager in Gainesville, FL. This full-time role involves close collaboration with physicians, employers, and patients to create effective treatment plans and ensure cost-efficient healthcare while promoting return-to-work goals.

Travel will be about 60%, with in-person visits required locally. A $2,500 sign-on bonus and a $2,500 retention bonus after one year are offered, along with a comprehensive benefits package and 24 days of vacation/holidays in the first year.

Qualifications

• Associates Degree or Bachelor’s Degree in Nursing or related field.
• 2+ years clinical practice preferred.
• URAC-recognized certification in case management (CCM, CDMS, CRC, etc.) preferred.

Responsibilities

• Demonstrate knowledge of case management standards of practice.
• Use advanced knowledge of injuries and treatments to advise on a plan.
• Interview disabled persons to assess recovery and occupational needs.
• Collaborate with physicians to create tailored treatment plans.
• Work with employers and physicians to modify job duties for early return to work.
• Evaluate and adjust case goals based on progress and effectiveness.
• Independently manage workload and prioritize cases.
• Attend appointments and prepare status updates for customers as needed.
