Job summary

Genex Service LLC is seeking a Field Case Manager in Rancho Cucamonga, CA to coordinate medical plans and return-to-work strategies. This full-time, remote role requires local travel for in-person appointments and offers a comprehensive benefits package.

You will work with physicians, employers and patients to design effective treatment plans, monitor progress, and adjust goals to ensure optimal recovery and work readiness. An active RN license is required.

Qualifications

• Education: Associates or Bachelor's in Nursing or related field.
• RN license active and in good standing.
• URAC-recognized case management certification (e.g., CCM, CDMS, CRC, CRRN or COHN/COHN-S, RN-BC, ACM, CMAC, CMC).
• 2+ years of clinical practice preferred; workers' compensation experience preferred.

Responsibilities

• Manage and coordinate treatment plans with physicians/providers and employers.
• Support early return-to-work by tailoring job duties with employers.
• Collaborate with clinicians and use available resources for patient care.
• Maintain timely updates and status reports for clients and stakeholders.
