Job summary

Genex Service LLC is seeking a Field Case Manager to coordinate case management for injured workers. This full-time, work-from-home role requires local travel for in-person patient appointments in Rancho Cucamonga, CA.

You will work with physicians and employers to develop and implement treatment plans, advocate for patients, and manage multiple cases efficiently from a home-based setting. A nursing license and relevant URAC certifications are preferred.

Qualifications

• Associates or Bachelor’s degree in Nursing or related field is required.
• 2+ years of clinical practice preferred; workers' compensation experience preferred.
• RN license in good standing required; URAC-recognized CCM/CDMS/CRC/CRRN/COHN/COHN-S/RN-BC/ACM/CMC etc. certifications preferred.

Responsibilities

• Demonstrate knowledge of case management standards of practice.
• Advise on treatment plans considering injuries, medications, and comorbidities.
• Interview injured workers to assess recovery and occupational status.
• Collaborate with physicians to implement tailored treatment plans.
• Modify job duties with employers to facilitate early return to work.
• Manage workload and prioritize cases.
• Attend appointments and prepare status updates as needed.
